Laparoscopy

Laparoscopic gynecological surgery is a minimally invasive approach that allows the surgeon to operate without making a large incision. A thin, lighted tube with a camera on the end, known as a laparoscope, is inserted into the abdomen through a small incision. The camera sends images of the inside of the body to a TV monitor in the operating room, allowing the surgeon to see and operate on the pelvic organs without having to use a long incision. Laparoscopic surgery can be used to treat a variety of gynecologic conditions that previously required large incisions, including endometriosis, fibroids, ovarian cysts, ectopic pregnancy, sterilization, pelvic problems such as urinary incontinence and pelvic support problems such as uterine prolapse. It can also be used for a variety of procedures, including a laparoscopic hysterectomy and a laparoscopically assisted vaginal hysterectomy.

What is Laparoscopy?

Laparscopy is an operation in which a telescope is inserted through the umbilicus (belly button to inspect the pelvic organs i.e. uterus (womb ).fallopian tubes and ovaries. Diagnostic laparoscopy issued to try and find a cause for symptoms. Laparoscopy may also be therapeutic or operative, when treatment is performed.

Who might benefit from laparoscopy?

Laparoscopy is useful in evaluating pelvic pain, painful periods, painful intercourse and infertility it is also used now a days for removing tubal pregnancy, fibroid cysts endometriosis and even for removal of the uterus (hysterectomies)

How is laparoscopic performed?

  • General anesthetic is administered, so the procedure itself is painless.
  • A small cut (0.5cm) is made inside/above the umbilicus.
  • A needle is introduced via the incision in to the abdominal cavity, which fills the abdomen with gas carbon dioxide).This allows the pelvic organs to seen more clearly.
  • The telescope (laparoscopic) to which a camera is attached is inserted.
  • Up to three other small incisions may be made to introduce other necessary instruments in to the abdominal cavity.

Which other Procedures may be performed during Laparoscopy?

  • Treatment of endometriosis – endometriosis occurs when issues which resembles the lining of the womb is found in other places.eg.in the pelvis .It may cause painful periods, the pain often starting before the period.
  • Dye test-this is used for women who are trying to conceive. Blue dye 9 which is harmless to you 0 is introduced into the womb from below .The dye should travel up the tubes and pill out of the ends in to the pelvic cavity
  • Adhesiolysis – adhesions may be caused by previous surgery or infections. Bands of fibrous scar tissue stick organs together and may cause pain
  • Removal/aspiration of ovarian cysts, removal of ovaries and fallopian tubes.
  • Removal of fibroids and other tumors.
  • Sterilization – family planning surgery
  • Hysterectomy – Removal of the uterus.

 What are the advantages of laparoscopy?

  • The smaller incision size also reduces pain after surgery. When a large incision has been made, patients usually require long term pain relief medication while the stich line heals. With laparoscopic surgery the post surgical wound is much smaller and the healing bleeding much less painful.
  • The smaller incision also leads to the formation of a significant smaller scar after surgery
  • Bleeding during surgery and blood loss is less than open surgery
  • Exposure of the internal organs to external contaminants is significantly reduced in laparoscopic surgery compared with open surgery, therefore reducing the risk of post operative infection
  • Patient go home faster with laparoscopic surgery, since healing is so much faster. Most patients receive a small day or next day discharge and can return to their normal everyday lives much more quickly than after an open surgery procedure.

The smaller incisions are less painful. As a result, we see a decreased need for pain medication, shorter recovery time and better cosmetic result. Patients are able to eat when they feel ready, and there is less internal scarring.

The risks are similar for laparoscopic, open and vaginal surgery. There is always a possibility that the surgeon cannot complete the procedure laparoscopically and a larger incision may be needed. This risk is small, as are the risks of bleeding, infection, damage to the bladder, bowel, blood vessels and ureters. With any surgery, there may be unforeseen risks and a potential, but exceedingly rare risk of death.
